Housing Application
1. Complete all areas of the rental application. Any area that does not apply should be marked "NA".
2. Complete the "Request for Rental History" section for each applicant age 18 or older. If additional forms are needed, please
contact our office. The following spaces must be filled in: date, landlord name, landlord address, and renter's signature. Two rental
references are required for each applicant 18 or older and must include applicant's current landlord.
3. Complete the "Request for Employment History" section for each applicant age 18 or older. If additional forms are needed,
please contact our office. The following spaces must be filled in: date, employer name, address, and employees signature.
4. Verification of monthly income. This can be in the form of your most recent pay stub or a letter from Social Security, Social
Security Disability, Social Services, Garrett County Community Action, Pension Office, Court Order Child Support, etc.
5. Complete the "Request for Personal References" section for each applicant age 18 or older. If additional forms are needed,
please contact our office. The following spaces must be filled in: date, references name, address, and applicant's signature.
6. The application, rental history, employment history, and personal references sections must be returned to our office with
your credit check fee of $25. An application will be processed ONLY IF it is fully completed and accompanied
by a credit check fee.
Upon receipt of your fully completed application and credit check fee, we will determine your eligibility. The process is usually completed
within four (4) to five (5) business days depending on the availability of your references. We will contact you by phone should you
be approved, and by letter in the event you are declined.

In the event you become seriously ill, are in jail or penitentiary, or if you die, access to your apartment WILL NOT be granted
without a court order or affidavit.

The undersigned warrants and represents above statements are true and correct and hereby authorizes verification of the above
information through all available means including but not limited to obtaining a consumer credit report and other reports as
maintained by City, County, State and Federal Law Enforcement Agencies, present and/or past employers including but not limited
to present and/or past salary verification, and present and/or past residences. Applicant agrees to furnish additional credit
references upon request.
Owner/Owner's agent reserves the right to regularly furnish information to consumer reporting agencies and other rental housing
owners about performances of lease obligations by our residents. Such information, which maybe be reported at any time, includes
both favorable and unfavorable information regarding resident's compliance with the lease, rules and financial obligations.
